Depending on the plan and the patient some common scenarios I see with nutrition coverage are as follows:

3 visits max plus additional visits if the patient has a referral or documentation of a medical condition (PCOS, Diabetes, High Cholesterol, BMI* related conditions, etc.). *For insurance purposes only; I have not found it useful to use BMI as a health indicator and problematic to focus solely on BMI and/or weight.

Even though I am out of network, some insurance plans may cover nutrition services (UHC has covered some sessions in the past)
Checking coverage for nutrition services:
I work closely with a billing company to check your nutrition related benefits and provide a cost estimation to you as best as possible. However we cannot 100% confirm the cost to you until the insurance claim is processed. There are times we are told one thing, but then something changes as the claim is processed despite our best efforts upfront. If there is a discrepancy, we will call again to try and figure out what happened.

Cancellation Policy
Please be mindful that I cannot bill your insurance for time we are not meeting face to face either in person or over Zoom. Please let me know at least 24 hours in advance if you need to reschedule or leave our session earlier than planned so I can plan accordingly. I understand that life happens and sometimes cancellations or rescheduling 24 hours in advance may not be possible once in a while.
If you need to cancel or reschedule your appointment, please do so by contacting me via email or phone at least 24 hours in advance of your appointment time. Appointments not canceled within this timeframe will be charged a flat fee of $25 dollars.
LATENESS Policy
Appointments will be held for 15 minutes. If you arrive within this time period, you will be seen, but your appointment time may not be extended. After 15 minutes, you will be charged a flat fee of $25 dollars.
